CCM helps patients with two or more chronic conditions stay healthier between visits. We identify and address care gaps proactively with regular monitoring and support so small problems don’t become big ones. We aim to prevent issues before they become larger problems.
What it looks like
Patients receive monthly check-ins and care coordination from our team. Practitioners connect via telemedicine several times a year to review progress, update care plans, and provide guidance and recommendations. All interactions are carefully documented to meet CMS requirements.
