Remote Patient Monitoring (RPM)
We take your vitals regularly and send them to our clinical team so they can keep an eye on you between doctor visits — all managed remotely. Our team visits Monday through Friday to take readings and report any concerns to the facility's clinical staff right away.
Chronic Care Management (CCM)
If you have two or more ongoing health conditions, our team works remotely each month to help coordinate your care and make sure nothing falls through the cracks. We send care gap reports to the facility so the staff here stays informed.
Behavioral Health Integration (BHI)
For residents dealing with stress, anxiety, depression, or other emotional health concerns, our clinical team supports you remotely and works with the facility to make sure your emotional wellbeing is part of your overall care.
How It Works for You
You're Approached for Enrollment
A member of our team will visit you to explain the programs and check whether your insurance qualifies. Enrollment is voluntary — we'll answer any questions you have before getting started.
We Enroll You
Once you agree to participate, we complete your enrollment on the spot. We confirm your coverage, set up your monitoring profile, and get everything in place before our first visit.
Monday–Friday Visits & Vitals
Our team visits Monday through Friday to take your vitals. Readings are reviewed by our clinical team and any concerns are reported to the facility's clinical staff the same day.
Care Gap Reporting to Your Facility
We send regular care gap reports to the facility's clinical team — keeping your nurses and doctors informed so small changes are caught early and unnecessary hospitalizations are avoided.
